Obtaining A Copy Of Your Child’s Immunization Record

Click on the this “Patient Portal” link or tab on the upper right and login. Go to “My Health Record” on the upper left and click on the drop down.
Select “Summary” and then go to the “Immunizations” box and click on the arrow on the upper right.
Click on the drop down and select “Administered Vaccinations” and you will be able to view and print the immunization record.
